Care Co-ordinator / Secretary – General Practice

Hours: 33–37.5 hours per week

Working Pattern: Monday to Friday between 08:00 – 18:30

Salary: Negotiable depending on experience

Location: General Practice

We are seeking a motivated and organised Care Co-ordinator/Secretary to join our friendly and supportive General Practice team. This is a key role within the practice, supporting both clinical staff and patients to ensure high-quality, coordinated care.

Main Duties & Responsibilities

· Act as a point of contact for patients, clinicians, and external services

· Support care coordination for patients with long-term conditions and complex needs

· Manage clinical correspondence, referrals, and reports

· Provide comprehensive secretarial and administrative support to the clinical team

· Maintain accurate patient records and ensure data quality

· Liaise with community services, hospitals, and other healthcare providers

· Support MDT working and care planning

· Ensure confidentiality and information governance standards are upheld at all times

Skills & Experience

Essential:

· Excellent organisational and time-management skills

· Strong communication skills (written and verbal)

· Ability to work independently and as part of a team

· Good IT skills and attention to detail

Desirable:

· Previous experience in General Practice or a healthcare setting

· Knowledge of clinical systems (EMIS, Docman)

· Understanding of care coordination, long-term conditions, or NHS pathways
